Introducción: la sinusitis odontogénica (SO) es una condición infradiagnosticada de la esfera otorrinolaringológica a pesar de su frecuencia que se estima entre 10 y el 40% de la rinosinusitis crónica. La SO representa hasta 75% de los casos de sinusitis maxilar unilateral y sigue pasando desapercibida en las guías más actuales de rinosinusitis, ocasionando una falta de consenso sobre los criterios diagnósticos y las pautas terapéuticas a observar. La dificultad en identificar el foco odontogénico en otorrinolaringología (ORL), y la de estimar la magnitud de la sinusitis en consultas de odontología, conduce frecuentemente a la persistencia de los síntomas y al fracaso de las terapias conducidas, impactando considerablemente en la calidad de vida de los pacientes. Por lo tanto, se elaboró esta revisión de la literatura para entender los desafíos que esta condición supone, a la luz de los estudios recientes en el tema. Métodos: se ha realizado una búsqueda exhaustiva de la literatura en Pubmed, Scopus y Google Scholar con términos relativos a las secciones y subsecciones de esta revisión. Resultados y conclusiones: el diagnóstico y el manejo de la SO plantean, por tanto, un desafío importante debido a la falta de protocolos estandarizados de diagnóstico y de procedimientos terapéuticos multidisciplinares consensuados. Se recomienda un enfoque interdisciplinar personalizado para lograr la resolución de la sintomatología y se precisan estudios bien diseñados, con estratificación según los causantes dentales y iatrogénicos, para generar una evidencia que respalde los futuros protocolos. Introducción: La alveolitis es una complicación tras una exodoncia dental quecursa con dolor intenso, trismo y mal olor.Esta complicación se relaciona con el hábito tabáquico, la higiene oral, no seguirlas normas post extracción o la edad yel género del paciente. Otras causas potenciales que podrían desencadenar esteproceso son el ciclo menstrual en el casode las pacientes de género femenino o eltratamiento con anticonceptivos orales. Elobjetivo de esta revisión es comparar, según la literatura, la incidencia de alveolitisen mujeres en tratamiento con anticonceptivos orales respecto a las que no lostoman y respecto a los hombres.Métodos: Se realizó una búsqueda bibliográfica en las bases de datos PubMed/Medline, Science Direct, GoogleScholar y Scopus y se incluyeron artículos en inglés y en español relacionadoscon el tema a tratar. Las palabras claveutilizadas fueron: “alveolar osteitis” OR “fibrinolytic alveolitis” OR “localized osteomyelitis” OR “delayed extraction woundhealing” AND “contraceptives” OR “OC”OR “contraceptive pill”. Se incluyeron estudios sobre exodoncias en pacientes entratamiento con anticonceptivos o sin anticonceptivos o varones, según los gruposde control que se han propuesto estudiar,que registrasen casos de alveolitis.Resultados: Se observó una mayor incidencia media de alveolitis post extracciónen las pacientes en tratamiento con anticonceptivos (18,52%), respecto a otraspacientes que no lo estaban (6,78%) yrespecto a la población de género masculino (6,4%)...(AU)


Introduction: Alveolitis is a complicationafter dental extraction which courses withintense pain, trismus, and halitosis. This complication is associated with smoking,oral hygiene, not following post-extraction rules or the age and gender of the patient. However, other potential causes could trigger this process, such as the menstrual cycle in the case of female patients or the fact that they are under treatment with oral contraceptives. The objective of this review was to compare the incidence of alveolitis among patients treated with oral contraceptives with those who do not take them. Methods: A bibliographic search was carried out in the databases PubMed/Medline, Science Direct, Google Scholar and Scopus and articles in English and Spanish related to the topic were included.Keywords used were: “alveolar osteitis ”OR “fibrinolytic alveolitis” OR “localized osteomyelitis” OR “delayed extraction wound healing” AND “contraceptives”OR “OC” OR “contraceptive pill”. Studies on exodontia in patients treated with contraceptives with registered cases of alveolitis were included.Results: A higher mean incidence of alveolitis was observed in patients treated with contraceptives (18.52%),with respect to other patients who were not (6.78%) and with respect to male population (6.4%)...(AU)

In this study, the aim was to evaluate the effects of the adjuvant piperacillin-tazobactam solution in the mechanical treatment of periodontitis. A single-blind split-mouth randomized study, it included 24 participants. All of them presented periodontitis stage III according to the 2018 World Workshop classification and the presence of at least one of the following periodontal pathogens: Aggregatibacter actinomycetemcomitans; Porphyromona gingivalis; Treponema denticola; Tannerella forsythia; Prevotella intermedia. The study established two groups: a control group (SRP: scaling and root planing) and a test group (SRP plus local piperacillin-tazobactam). The final recruitment included 11 women (45.8%) and 13 men (54.2%). The age range was between 25 and 72 years, and the mean age was 57 ± 10.20 years. Clinical controls were performed at 2 weeks, 3 months, and 6 months, repeating the SRP and applying the piperacillin-tazobactam solution again at the 3-month appointment. The clinical attachment level decreased by a mean of 2.13 ± 0.17 mm from the baseline to 6 months in the test group versus 1.63 ± 0.18 mm in the control group. The mean probing pocket depth decreased from 1.32 ± 0.09 mm in the test group, versus from 0.96 ± 0.14 mm on the control side. The plaque index in the test group decreased by 0.46 ± 0.04, while it decreased by an average of 0.31 ± 0.04 in the control group. In conclusion, the local use of piperacillin-tazobactam as complementary therapy produces better clinical results in patients with periodontitis. However, these results are not maintained over time, and so a more persistent local application is necessary.

The aim of this study was to analyze the prevalence of transmigrated canines in a Spanish population by evaluating their clinical and radiological characteristics. The descriptive observational study obtained 6840 orthopantomographs from all patients seeking dental care in the years 2017-21 via the Patient Reception Service and Dentistry Service at the Faculty of Dentistry at the Complutense University of Madrid and the Central Hospital of the Red Cross of Madrid (Spain). In total, 52 patients presented transmigrated canines, establishing a prevalence of 0.76%. This sample comprised 28 women and 24 men. Whenever a transmigrated canine was identified, a CBCT scan was obtained and used to evaluate the clinical and radiological variables associated with canine transmigration. The predominant side of the transmigration was the left (57.69%) compared to the right side (42.30%). The position of the canine, in order of frequency, according to the Mupparapu classification, corresponded to type IV (42.30%), type II (36.53%), type I (15.38%), and type V (5.76%), with no type III transmigrations found. Clinical manifestations were only recorded in 17.30% of cases, and 11.53% of the radiological findings showed the presence of tooth cysts that were confirmed by histopathological studies. Other impactions, in addition to the transmigrated canine, were found in five patients (9.61%), with the majority being the presence of third molars.

PURPOSE: This study used cone-beam computed tomography (CBCT) to analyze the prevalence of several maxillary anatomical/accessory structures, as well as variations within each type, assessing how accurate diagnosis can minimize the risk of intraoperative complications during implantological procedures in the oral cavity. METHODS: 212 CBCT scans of the maxilla were analyzed, captured over a period of 18 months for surgical planning purposes. The prevalence of posterior superior alveolar arteries (PSAA), maxillary sinus septa (MSS), and branches of the canalis sinuosus (CS) were evaluated, as were the diameter and location of each anatomical structure in horizontal and vertical planes. P < 0.05 was considered statistically significant. RESULTS: PSAAs were observed in 99.1% of cases, the intrasinus type being the most frequent; MSS were noted in 15.6% of the sample, mainly in the posterior region with sagittal orientation; CS branches were observed in 50% of patients, mainly in relation to the incisors and significantly more prevalent among males. CONCLUSIONS: The use of CBCT significantly increases the possibility of clearly identifying these anatomical structures. The differences found between patients highlight the importance of carrying out an exhaustive radiological study of the individual to prevent complications, such as Schneiderian membrane perforation, neurovascular damage or bleeding during surgery.

Introducción: el desarrollo de los implantes dentales ha hecho que los autotrasplantes se realicen con menor frecuencia, pese a sus altas tasas de éxito y desuponer una alternativa terapéutica quepermite la realización de rehabilitacionesnaturales, funcionales y estéticas.Caso clínico: se presenta el caso de unvarón de 28 años, que acudió a consulta por la presencia de unos restos radiculares en localización de primer molarsuperior izquierdo. Se realizó un autotrasplante dental, siendo el diente donanteel tercer molar superior izquierdo, quese trasplantó al alveolo del primer molar,mostrando buena evolución clínica y radiográfi ca.Discusión: existen diferentes alternativas terapéuticas para reponer los dientesausentes, dentro de las cuáles se encuentran los autotrasplantes, con altas tasasde éxito en dientes con ápices abiertos ycerrados. Los dientes más frecuentemente trasplantados son los terceros molares,como se muestra en el presente caso clínico, sugiriéndose como una opción más,que deben tener en cuenta profesionalesy pacientes.Conclusiones: aunque los autotrasplantes tienen indicaciones más limitadas, suponen un menor coste para los pacientes,y una restauración más simple para elprofesional. Además, sufren micromovimientos, consiguiendo una armonía oclusal en relación a los dientes adyacentes,presentando una alta tasa de éxito (AU)


Introduction: dental autotransplants arenot performed frequently due to dentalimplant development, although theiracceptable success rates, and being atherapeutic option which allows natural,functional and aesthetic restorations.Clinical case: a clinical case of a 28-yearold man is presented, who went todental clinic for removal of left fi rst molarroots. An autotransplant was performedusing left third molar as donor tooth,which was transplanted into fi rst molaralveolus, showing promising clinical andradiographic evolution.Discussion: there are different therapeuticalternatives to restore missing teeth,being one of them dental autotransplants,with acceptable success rates in teethwith both opened and closed apexes.Third molars are the most frequentlytooth being transplanted, as shown in thisclinical case, being suggested as anotheroption, important for professionals andpatients.Conclusion: althouth autotransplants´indications are more limited, they aremore economical, and are simpler tobe restored. Besides, they suffer micromovements, having an occlusal harmonyin relation to adjacent teeth, with highsuccess rates(AU)

The aim of this study was to identify the most relevant dental factors and iatrogenic causes in the development of pathological changes to the sinus membrane and to analyse their possible influence on the development of odontogenic sinusitis. A descriptive, observational study was designed, with 276 patients who had been evaluated via cone beam computed tomography, analysing possible sinus thickening factors, such as apical infections, endodontic treatments, periodontitis, radicular cysts and impacted teeth, as well as iatrogenic factors caused by implant treatments or the development of oroantral communications produced during tooth extraction manoeuvres. Among the dental factors, periodontitis (47.1%), apical pathology (23.5%) and endodontic treatments (23.1%) were the predominant causes of sinus membrane thickening that most frequently produced an occupancy between 2 and 10 mm. Regarding the implant treatments, the placement of implants through the floor of the maxillary sinus was the main cause (9.8%), followed by sinus elevation techniques (6.2%). Dental extraction was the first cause of oroantral communication (5.0%), being the procedure that caused the greatest thickening of the sinus membrane. This study highlights the importance of dental treatments and iatrogenic factors in sinus pathology, and the need for diagnostic interrelations between the different specialists who address this pathology.

Introducción: el empleo de técnicas regenerativas tras la extracción de dientes retenidos ha mostrado su eficacia con diversos materiales de regeneración, al asociarse al empleo de membranas, pero en los últimos años se ha descrito el empleo de dentina autógena en procedimientos de regeneración ósea, mostrando unos resultados favorables. Caso clínico: se presenta un caso clínico de una mujer de 20 años, sin antecedentes médico-quirúrgicos de interés, que acudió a consulta remitida por su ortodoncista, para la extracción de un premolar maxilar retenido, en posición invertida. Tras la extracción del premolar y del tercer molar inferior retenidos, se obtuvo dentina procedente de ambos, para la regeneración ósea guiada del defecto resultante de la extracción del premolar, realizando revisiones a la semana y a los 4 meses de la intervención. Conclusiones: la dentina autógena demuestra ser un biomaterial biocompatible, adecuado para procedimientos de regeneración ósea guiada, tras la extracción de dientes retenidos, como alternativa a otros biomateriales. Introduction: regenerative bone techniques after impacted teeth extraction have shown to be effective with different materials, when used in conjunction with membranes, but in the last few years, autogenous dentin has been used in bone regeneration procedures, having promising results. Clinical case: a clinical case of a 20-year-old woman is presented, with no interesting medical record, who went to dental clinic for removal of an inverse maxillary bicuspid retained. After maxillary bicuspid and lower third molar extractions, autogenous dentin was obtained from both teeth, to perform a guided bone regeneration of the bicuspid defect. A week and 4-month check-up were carried out. Conclusion: autogenous dentin has shown to be a biocompatible material, adequate for guided bone regeneration procedures, after extraction of impacted teeth, as an alternative to other biomaterials. Introducción: La participación en el rugby se ha visto incrementada con el paso de los años, especialmente en países donde la práctica de este deporte no era habitual, lo que ha arrojado como consecuencia un aumento en la incidencia de lesiones, dada la naturaleza de alto impacto del mismo. Diversos estudios epidemiológicos han evaluado la incidencia de traumatismos bucodentales como consecuencia de accidentes deportivos, en los que se hace referencia a las fracturas dentarias como aquellas más comúnmente producidas dentro de la práctica de este tipo de deportes. Descripción del caso clínico: Se presenta el caso clínico de un paciente varón de 18 años de edad, sin antecedentes médicos de interés, remitido al servicio de Cirugía Bucal del Hospital Viamed Virgen de la Paloma de Madrid, quién presentaba una lesión de tejidos blandos, además de luxación palatina del sector dentario anterosuperior, como consecuencia de un traumatismo durante un partido de rugby, para el cual se prescribió tratamiento farmacológico combinado con una ferulización, permitiéndose así la cicatrización espontánea de los tejidos. Discusión y conclusiones: Se observa una alta incidencia de lesiones producidas durante la práctica del rugby debido al escaso equipamiento protector usado por los jugadores, donde la cara y sus tejidos blandos constituyen las zonas con mayor frecuencia de afectación, pudiendo considerarse como el deporte de alto impacto con mayor prevalencia de lesiones de cabeza y cuello. Introduction: Participation in rugby has increased over the years, especially in countries where the practice of this sport was not common, which has resulted in an increase in the incidence of injuries given its high-impact nature. Various epidemiological studies have evaluated the incidence of oral trauma as a result of sports accidents, in which dental fractures are referred to as those most commonly produced within the practice of this type of sports. Description of the clinical case: We present the clinical case of an 18-yearold male patient, with no relevant medical history, referred to the Oral Surgery Service of the Virgen de la Paloma Viamed Hospital in Madrid, who presented a soft tissue lesion in addition to a palatal dislocation of the dental sector anterosuperior as a result of trauma during a rugby match, for which drug treatment combined with splinting was prescribed, thus allowing spontaneous tissue healing. Discussion and conclusions: A high incidence of injuries produced during rugby practice is observed due to the scarce protective equipment used by the players, where the face and its soft tissues constitute the areas with the highest frequency of affectation and can be considered as the high-impact sport with the highest prevalence of head and neck injuries. BACKGROUND AND OBJECTIVES: The main purpose of this study was to evaluate the survival and success rates of dental implants with a double acid-etched surface treatment with evaluation times up to 10 years post-loading. Materials and Methods: This study was conducted at a hospital oral surgery and implantology unit. It included 111 dental implants with a double acid-etched surface. Three groups were created: Group 1 (1-3 years loading), Group 2 (3-5 years loading), and Group 3 (over 5 years loading). Probing depth, resonance frequency analysis (ISQ value), and marginal bone loss were evaluated. Results: The data obtained underwent statistical analysis. Overall, 78 patients were included in the study, who received, in total, 111 dental implants, all replacing single teeth. Mean probing depth was 3.03 mm and mean ISQ was 65.54. Regarding marginal bone loss, in Group 1, 67.6% of implants did not undergo any thread loss, in Group 2, 48.3%, and in Group 3, 59.6%; 59.10% of all implants did not present thread loss with a mean bone loss of 0.552 mm. The implant survival rate was 99.1%, and the success rate was 96.37%. Conclusions: Implants with a double acid-etched surface showed excellent success rates in terms of marginal bone loss, ISQ, and probing depth after up to 10 years of loading, making them a clinically predictable treatment option. Future studies are needed to compare this implant surface with other types in different restorative situations.

BACKGROUND: Nonsurgical treatment, resective surgery, reconstructive surgery, or combined approaches have been proposed for the treatment of peri-implantitis, with variable results. PURPOSE: To evaluate the 1-year clinical and radiographic outcomes following combined resective and reconstructive surgical treatment with topical piperacillin/tazobactam antibiotic in the management of peri-implantitis. MATERIAL AND METHODS: Forty-three patients diagnosed with peri-implantitis were included. Surgical treatment consisted of implantoplasty of the supra-crestal component of the defect, the application of a topical antibiotic solution over the implant surface, and subsequent reconstruction of the intra-osseous component of the peri-implant defect. The primary outcome was disease resolution, defined as the absence of bleeding on probing (BoP) and/or suppuration on probing (SoP), a peri-implant pocket probing depth (PPD) ≤5 mm, and no bone loss >0.5 mm 1 year after surgery. Secondary outcomes included changes in BoP, PPD, SoP, and peri-implant marginal bone levels. One implant per patient was included in the analysis. RESULTS: The treatment success rate of the 43 dental implants included in the study was 86% at 1 year after surgery. Mean PPD and BoP decreased from 6.41 ± 2.11 mm and 100% at baseline to 3.19 ± 0.99 mm (p < 0.001) and 14% (p < 0.001) at 1 year, respectively. SoP was significantly reduced from 48.8% at baseline to 0% 1 year after surgery (p < 0.001). Radiographically, a mean defect fill of 2.64 ± 1.59 mm was recorded (p < 0.001). CONCLUSIONS: The combination of a resective and reconstructive surgical approach together with locally delivered antibiotic achieved a high disease resolution rate after 1 year of follow-up and constitutes a viable option for the management of peri-implantitis.

After tooth extraction, the alveolar bone undergoes a physiological resorption that may compromise the future placement of the implant in its ideal position. This study evaluated bone density, morphological changes, and histomorphometric results undergone by alveolar bone after applying a new biomaterial composed of calcium phosphate modified with silicon (CAPO-Si) compared with hydroxyapatite of bovine origin (BHA). Alveolar ridge preservation (ARP) was performed in 24 alveoli, divided into a test group filled with CAPO-Si and a control group filled with BHA. Three months later, the mineral bone density obtained by the biomaterials, horizontal and vertical bone loss, the degree of alveolar corticalization, and histomorphometric results were evaluated. Both biomaterials presented similar behavior in terms of densitometric results, vertical bone loss, and degree of alveolar corticalization. Alveoli treated with CAPO-Si showed less horizontal bone loss in comparison with alveoli treated with BHA (0.99 ± 0.2 mm vs. 1.3 ± 0.3 mm), with statistically significant difference (p = 0.017). Histomorphometric results showed greater bone neoformation in the test group than the control group (23 ± 15% vs. 11 ± 7%) (p = 0.039) and less residual biomaterial (5 ± 10% vs. 17 ± 13%) (p = 0.043) with statistically significant differences. In conclusion, the ARP technique obtains better results with CAPO-Si than with BHA.

Las malformaciones dentales son defectos formativos causados por alteraciones genéticas durante la morfogénesis den-tal. Pueden ser provocados por factores de índole hereditario, sistémico, traumático o local. Dentro de estas anomalías se encuentra el taurodontismo. Éste se caracteriza por presentar una alteración de la morfología dentaria en la que la porción coronaria del órgano dentario se encuentra alargada con una cámara pulpar ensanchada, a expensas de la porción radicular, disminuyendo la longitud y por tanto, produciéndose una migración apical de la furca del diente. El objetivo del presente artículo es describir las anomalías de forma dentaria y más concretamente el manejo clínico del paciente con taurodoncia en la clínica dental. Para ello se presenta el caso clínico de un paciente de 18 años de edad que acude al Hospital Universitario de la Princesa de Madrid para valorar la exodoncia de dos molares localizados en el primer y cuarto cuadrante. Tras un diagnóstico clínico y radiológico apropiado, se procedió a la exodoncia de los mismos


Dental anomalies are formative defects caused by genetic disturbances during tooth morphogenesis, can be caused by factors of a hereditary, systemic, traumatic or local nature. One such anomaly is taurodontism. It is characterized by pulp chamber enlargement, which may approximate of the root apex, with the body of the tooth enlarged at the expense of the roots and apically displaced furcation areas.The aim of this case report is to describe the abnormalities of tooth shape and particularly the management in the dental clinic of patients with taurodontism. For this purpose a case of a eighteen years patient visiting the Hospital Universitario de la Princesa of Madrid to assess the extraction of two molars located in the first and fourth quadrants. Background: To establish the prevalence of supernumerary canines (SNC) in a sector of the population of Madrid (Spain), as well possible complications associated with this unusual developmental variation. Study design: This observational study was performed between 2005 and 2017, among 21,615 patients seeking dental treatment at the Faculty of Dentistry, Complutense University of Madrid (Spain), and at the Virgen de la Paloma Hospital, Madrid (Spain); 22 patients with 26 SNCs were diagnosed. These 22 patients underwent clinical and radiological exploration, registering patient data. Results: SNCs presented a prevalence of 0.10% of the study population. The supernumerary teeth (SNT) were located in the upper maxilla more frequently (61.54%) than the mandible (38.46%). 69.23% were found to be impacted, also causing the impaction of the permanent canine in 53.85% of these cases. In 15.38%, follicular expansion > 3mm was observed. SNCs were associated with other SNT in only four patients. Conclusions: Despite of the fact that the SNCs are usually diagnosed casually in the course of radiological explora-tion, in the present study over half of them (53.85%) caused impaction of the permanent canine. Early diagnosis allows optimal patient management and treatment planning, with intervention at an appropriate time to prevent complications in development and so reduce later treatment need

Introducción: El Granuloma periférico de células gigantes (GPCG) es una lesión exofítica poco frecuente de la cavidad oral, cuya aparición se encuentra generalmente asociada al diente natural y cuya etiología se relaciona, entre otros factores, con el efecto crónico en el tiempo de diversos irritantes locales. Con el creciente uso de los implantes dentales, se ha observado la aparición de este tipo de lesiones en la mucosa periimplantaria, lo que obliga a estudiar y comprender su etiología y evolución en función de garantizar la longevidad de este tipo de rehabilitaciones. Caso clínico: Se presenta el caso de una paciente mujer de 41 años de edad, sin antecedentes médicos de interés, remitida al servicio de Cirugía Bucal del Hospital Virgen de la Paloma, la cual presenta una tumoración de base sésil asociada a un implante en posición de 47, de 1,2x0,6 cm, indolora, de consistencia firme y no desplazable con coloración congestiva. Se llevo a cabo un tratamiento de exéresis total de la lesión, remitiendo la pieza a estudio histopatológico, tras el cual se obtuvo el diagnóstico de GPCG. Conclusiones: Es de vital importancia mantener un control exhaustivo de los diversos factores locales que pueden identificarse como irritantes para los tejidos periimplantarios, como es el caso de una higiene oral deficiente, entre otros, con el fin de prevenir la aparición de lesiones que pudieran comprometer la supervivencia y éxito de los implantes a largo plazo


Introduction: The peripheral giant cell granuloma (GPCG) is a rare exophytic lesion of the oral cavity, whose appearance is usually associated with the natural tooth and whose etiology is related, among other factors, to the chronic effect over time of various local irritants. With the increasing use of dental implants, the appearance of this type of lesions in the peri-implant mucosa has been observed, which requires studying and understanding its etiology and evolution in order to guarantee the longevity of this type of rehabilitation. Clinical case: We present the case of a 41-year-old female patient, with no medical history of interest, referred to the Oral Surgery Department of the Hospital Virgen de la Paloma, which presents a sessile base tumor associated with an implant in position 47, of 1.2x0.6 cm, painless, firm consistency and nondisplaceable with congestive coloring. A total exeresis treatment of the lesion was carried out, sending the piece to histopathological study, after which the diagnosis of GPCG was obtained. Conclusions: It is vital to maintain an exhaustive control of the various local factors that can be identified as irritants for peri-implant tissues, such as poor oral hygiene, among others, in order to prevent the appearance of injuries that could compromise survival and success of long-term implants

El angioedema hereditario (AEH) es una enfermedad genética rara de transmisión autosómica dominante, en la que existe un déficit de la encima C1-INH. Cursa con episodios recurrentes y autolimitados de edema, causados por aumento de la permeabilidad vascular. Tiene unas importantes implicaciones para los profesionales de la salud bucodental, ya que determinados procedimientos odontológicos y quirúrgicos pueden desencadenar episodios de angioedema, potencialmente mortales al producirse edema en las vías respiratorias superiores. El objetivo de este trabajo es aportar información y recomendaciones a los odontólogos a la hora de tratar a estos pacientes con el fin de minimizar las complicaciones. Los resultados han aportado un total de 48 pacientes y se realizaron un total de 90 intervenciones, siendo el tratamiento más frecuente las extracciones dentales. El AEH tipo I es el más frecuente de los tres tipos con una incidencia de entorno al 85% y el tipo III es el menos común y conocido, ya que se ha identificado en los últimos años. La profilaxis a corto plazo es un método preventivo que ha de realizarse en todos los pacientes con AEH antes de cualquier intervención quirúrgica odontológica. El tratamiento de estos pacientes implica, en la mayoría de las ocasiones, una profilaxis a largo y corto plazo con el fin de disminuir los ataques. Siendo aconsejable realizar los procedimientos odontológicos-quirúrgicos en ambiente hospitalario


Hereditary angioedema (HAE) is a rare genetic disease of autosomal dominant transmission, in which there is a deficit of C1-INH enzyme. It runs with recurrent and self-limited episodes of edema to increase vascular permeability. There are some important complications for oral health professionals because many dental and surgical procedures can trigger angioedema episodes which can be a potentially life-threating condition due to edema in the upper respiratory tract. The objective of this research is to provide information and recommendations to dentists for treating these patients in order to minimize complications. 48 patients have been reported and a total of 90 interventions were performed, being extractions the most frequent treatment. HAE type 1 is the most frequent of the three types with an 85% of incidence, and type III is the least common and known, it has been identified in recent years. Shortterm prophylaxis is a preventive method that must be done in all HAE patients before any dental surgery. The treatment of these patients implies, in most cases, a long- and short-term prophylaxis in order to reduce attacks. It is recommended to realize dental-surgical procedures in a hospital environment

Introducción: La sialolitiasis parotídea es una patología común de las glándulas salivales y puede dar lugar a atrofia glandular. Los sialolitos parotídeos son más raros que los submandibulares y su diagnóstico y tratamiento es más complejo, debido a la superposición de estructuras y a las dificultades de acceso. Caso clínico: Mujer, de 68 años, que acude con episodios inflamatorios recurrentes en la región geniana izquierda, coincidente con las comidas, y que, tras su paso por varios especialistas, acude a nuestro Servicio, en el que tras prescribir un CBCT con reconstrucción tridimensional se halla una pequeña estructura calcificada próxima a la parótida izquierda. Discusión: Los métodos de diagnóstico son variados y su utilización depende de la indicación. Entre las distintas pruebas figuran la resonancia magnética (RM), laa Tomografía Computerizada de Haz de Cono (CBCT) y la sialografía, siendo el método más eficaz de diagnóstico la combinación de estas. Los abordajes terapéuticos son diversos y dependen de multitud de factores, siendo los más utilizados la sialoendoscopia y el abordaje quirúrgico transoral. Conclusión: La sialolitiasis parotídea supone un reto, tanto diagnóstico como terapéutico, y es una situación clínica común por lo que en su tratamiento deben tenerse en cuenta muchos factores para elegir la alternativa que suponga una remoción completa del cálculo y pocas complicaciones a largo plazo


Introduction: Parotid stones are a common finding in salivary gland disorders and can lead to destruction of salivary tissue. Parotid calculi are rarer than submandibular calculi and their diagnosis and treatment are more challenging due to superimposition of hard structures and limited access. Clinical case: A female patient, 68 years old, is referred to our Service, after seeing different specialists, due to repeated inflammatory episodes of her left cheek area, which usually appear at mealtimes. After prescribing a CBCT with 3D reconstruction, a calcified structure can be seen next to the left parotid gland. Discussion: Diagnostic means are varied, and their use depends on their indication. Among diagnostic test, MRI, CBCT and sialography can be found, and usually combining them provides a more precise diagnosis. There are many therapeutical approaches, being sialoendoscopy and transoral approach the most common ones. Conclusion: Parotid sialolithiasis poses a diagnostic and therapeutic challenge, and it is a relatively common clinical situation, thus, treatment must be tailored to obtain complete stone removal and a minimum of long-term complications

Introducción: La falta de volumen óseo en el maxilar es un hallazgo habitual en los pacientes que presentan pérdidas dentarias de larga duración. En la actualidad el uso de implantes es la opción más demandada para su rehabilitación, pero para ello son necesarias unas magnitudes mínimas y en los casos que no se presenten, habrá que realizar técnicas de regeneración ósea para que el tratamiento sea viable. Caso clínico: Paciente varón de 58 años de edad que acudió a Servicio de Cirugía Bucal e Implantología del Hospital Virgen de la Paloma de Madrid. Fue diagnosticado hace 7 años de enfermedad periodontal crónica del adulto; tras un periodo de abandono, regresó para la valoración de una rehabilitación completa superior. Se realizó una minuciosa exploración intraoral. Como pruebas complementarias se realizaron una radiografía panorámica y, posteriormente, un estudio tomográfico, donde se observó un insuficiente volumen óseo para el tratamiento implantológico, por lo que se planificó realizar una elevación sinusal bilateral mediante ventana con un injerto autógeno de calota combinado con plasma rico en plaquetas. Conclusiones: El tratamiento de elevación sinusal mediante ventana con injerto de calota es una opción a tener en cuenta para la rehabilitación impantológica de la arcada superior


Introduction: The lack of maxilla bone volume makes the implant treatment difficult. In order to make the implant rehabilitation possible a certain hight is required, which is not enough in this particular case. Therefore a regular sinus lift with lateral approach. Currently, the use of implants is the most demanded option for rehabilitation, but for this minimum magnitudes are necessary and in cases that do not occur, bone regeneration techniques must be performed in order for the treatment to be viable. Clinical case: Male 65 years old patient, arrived at the Oral Surgery service at the Virgen de la Paloma Hospital. No medical or family record of interest. Diagnosed seven years ago with a periodontal chronic disease. After a period of not taking care of it, he returned a few months ago with severe tooth mobility and bone loss, for a full rehabilitation valuation.A meticulous oral inspection was performed. As additional tests, a panoramic radiograph was performed as well as a tomographic test. As a result it was confirmed that the maxilla bone volume was not enough to develop any implant treatment. Therefore it was decided to perform a bilateral sinus lift via a calvarial graft mixed with plateletrich plasma at in hospital environment. Conclusion: The sinus lift treatment via craneal calvarial graft combined with platelet-rich plasma is an option to keep in mind for the full implant rehabilitation of the upper dental arch

La transmigración se define como el fenómeno en el cual un diente incluido no erupcionado, traspasa en más de la mitad de su longitud la línea media. Es una anomalía poco frecuente y es aún más raro que ocurra de forma bilateral. Se presenta con mayor incidencia en los caninos mandibulares, en el sexo femenino, de forma unilateral, y siendo el canino izquierdo el más comúnmente afectado. El objetivo de este trabajo es describir el manejo quirúrgico satisfactorio en un paciente varón de 16 años, asintomático, el cual presentaba a la exploración clínica intraoral ausencia de ambos caninos mandibulares permanentes y persistencia de canino temporal inferior izquierdo, acompañado de una ligera malposición dentaria. En la exploración radiográfica (panorámica), se observaron ambos caninos inferiores derecho e izquierdo, en posición horizontal, bajo los ápices de incisivos inferiores. Se procedió a la extracción quirúrgica de ambos caninos, transcurriendo el postoperatorio sin incidencias. Se realizaron controles periódicos y radiográficos satisfactorios a los seis meses. Transmigration is defined as the phenomenon in which an unerupted included tooth tran-scends the midline by more than half of its length. It is a rare anomaly and it is even more rare to occur bilaterally. It is presented with greater incidence in the mandibular canines, in the female sex, unilaterally, and the left canine being the most commonly affected. The objective of this study is to describe satisfactory surgical management in a 16-year-old asymptomatic male patient, which presented to the intraoral clinical examination absence of both permanent mandibular canines and temporary biscuspid persistence. Lower left, accompanied by a slight dental malposition. In the radiographic exploration (panoramic), both right and left lower canines were observed, in a horizontal position, under the lower incisor apexes. We proceeded to the surgical extraction of both canines, passing the post-operative period without incidents. Satisfactory periodic and radiographic controls were performed at six months.
